A Scoping Review of "Discharge Before Noon" Interventions

This scoping review examined “discharge before noon” (DBN) interventions to identify which components are associated with success. The authors systematically searched the literature and found 15 studies that both described and evaluated a DBN intervention. Of these, 13 studies reported improved DBN.<br /><br />Using cross-case analysis, the review found that effective interventions consistently shared certain features. The most important were: multidisciplinary team involvement, a structured communication platform for discharge planning, and pre-identification of patients and potential barriers to discharge. Compared with ineffective interventions, effective ones much more often included these elements.<br /><br />The study also found that simply changing workflow timing, such as making physician rounds earlier, was not enough on its own to improve DBN. While this strategy appeared in many effective interventions, it was also present in all ineffective ones, suggesting it may support success but cannot drive it alone.<br /><br />Overall, the review highlights that DBN improvement likely depends on combinations of intervention components rather than any single strategy. The authors conclude that DBN programs should be tailored to local context and designed as coordinated, team-based processes with clear communication and proactive discharge planning. These findings may help hospitals implement more effective DBN interventions and better use resources.
